COVID-19 FAQ

A Michigan legislation was passed that appropriated funding for the Teacher COVID-19 Grant and School Support COVID-19 Grant Programs.  These programs were created to recognize school staff for their efforts to provide continuing education during the COVID-19 pandemic.  You are receiving this letter and the attached Form 5734 because you may qualify to receive one of these payments.

 

 Please read the attached instructions carefully to see if you qualify for one of the grant payments.  If you believe that you meet the qualifications, please complete the attached form and certify your eligibility.  Return the completed form to your school by December 4, 2020.
